The New York Care Coordination Program (NYCCP) is featured this month’s  Agency for Healthcare Research and Quality’s Innovations Exchange.  The article "Care Coordinators Support Individuals with Severe Mental Illness, Leading to Improved  Quality of Life and Lower Costs" illustrates the effectiveness of local, on-the-ground behavioral health-based care coordination as a way to lower costs and improve quality of life for people with severe mental illness.   CCSI provides the New York Care Coordination Program with a range of essential support services, including: project management staffing, information technology and data analysis, as well as financial management, including budget development, monitoring, and funds management.
