Our NY State Trained and Certified Navigators will assist you with applying for health insurance through 
the New York State of Health Marketplace.

You must bring the following items for each member in your household:
  • Social Security Numbers
  • Birth Dates
  • Citizenship Documents, Driver License, Permanent Resident Cards
  • Income verification for the last 4 weeks
  • 2017 Federal Tax return if self-employed
  • Available Employer Based Health Insurance or any other insurance information
     
 

Funded by the New York State Department of Health, the IPA / Navigator program provides one-on-one assistance for individuals and small businesses interested in shopping for and enrolling in health insurance through The New York State of Health - the Official Health Plan Marketplace. Navigators are now available to meet with individuals, families and small businesses at various locations throughout the community to provide education about the different types of health insurance plans offered through the New York State of Health marketplace and to assist with enrollment into public insurance programs and qualified health plans.

CCSI is providing support at 13 locations across the Monroe and Livingston county area, with many locations offering early morning and/or evening hours.

Open Enrollment through the NY State of Health may have ended, but did you know you can apply anytime for:
  • CHILD HEALTH PLUS
  • MEDICAID
  • THE ESSENTIAL PLAN
  • Or if you have had a QUALIFYING LIFE EVENT within 60 days
