Family Support Center Programs

Family Access and Connection Team (FACT)

FACT's goal is to partner with families to prevent higher-level system involvement. FACT engages with families of children and youth with high needs to set goals and work through challenges using natural supports and our collective knowledge, experience, and connections. Our goal is to help youth and families increase hope, believe that tomorrow can be better, and know they can impact their future. FACT is short-term intervention that includes conducting strength-based assessments and making referrals to service providers.

Youth and Family Partnership (YFP)

YFP assembles Child and Family Teams (CFT) to support youth and their families in addressing and managing their challenges. Child and Family Team Specialists meet with the entire family to learn about their strengths, interests, talents, and needs to build a support team. Team members may include parents, guardians, family supports, health care providers, mentors, community resource providers, teachers, and faith leaders. The CFT creates an individualized plan that allows the youth and family to develop strategies and supports to address and manage challenges, creating lasting change.

Referrals are only accepted from Child Protective Services, FACT and the Office of Mental Health.

Family Voice

Family Partners encourage and assist families of children under the age of 21 with mental, emotional, and behavioral problems. Partners offer support and promote activities to reduce isolation, increase hope, and strengthen the family voice. Family support includes individual support and Family Voice Meetings.

The Family Support Center is a Monroe County initiative supported by CCSI in partnership with the Monroe County Department of Human Services, Office of Mental Health and the Office of Probation/Community Corrections.
