Health Home Children’s Care Management Supervisor

We are currently seeking a Children’s Care Management Supervisor to support our Health Home Care Management program. Liberty Resources’ Care Management provides tailored and individualized supports to meet the needs of children, families and individuals. Our Care Managers create open lines of communication between different service providers to avoid unnecessary duplication of services and improve the health and well-being of those we serve.

The Care Management Supervisor is responsible for the day-to-day supervision of Care Managers, and is the staff in charge in their absence, both in the office and in the community.
Supervises staff in the day-to-day implementation of program services, and provides support and direction to achieve program deliverables and encourage staff performance.
Conducts ongoing review of documentation and statistics, and utilizes data to ensure that the program delivers its intended results, and also meets all contractual, regulatory, and other specific standards.
Develops, implements, communicates and models expectations to staff to ensure quality services are being delivered.
Participates in the recruitment and onboarding of Care Managers, and develops and facilitates training to ensure skill development of staff.
Builds and maintains relationships with internal and external stakeholders.
Responds to and resolves client and program-related emergency situations.
Periodic travel outside of assigned county as necessary
Bachelor’s Degree in health or human services field; master’s degree preferred.
LMSW, LCSW or LMFT preferred.
At least two years of supervisory experience is preferred.
Must have a valid New York State driver’s license and access to reliable transportation.
