POSITION: Health Home Care Manager

REQUIREMENTS:

Education: A Bachelor´s degree in any of the following: child & family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation, recreation therapy, rehabilitation, social work, sociology, or speech and hearing; OR NYS licensure and current registration as a Registered Nurse and a bachelor´s degree; OR A Bachelor´s level education or higher in any field with five years of experience working directly with persons with behavioral health diagnoses; OR A Credentialed Alcoholism and Substance Abuse Counselor (CASAC).

Experience: Two years of experience (a Master´s degree in a related field may substitute for one year´s experience) either: Providing direct services to persons diagnosed with mental disabilities, developmental disabilities, alcoholism or substance abuse; OR Linking persons who have been diagnosed with mental disabilities, developmental disabilities, alcoholism or substance abuse.

DUTIES. This position is responsible for providing direct care management services primarily to eligible clients reentering the community from NYS prisons, along with other clients as assigned. Health Home Care Managers coordinate various aspects of client care with outside providers, including chemical dependency, mental health and medical services, accessing benefits (Medicaid, food stamps, etc.), locating safe housing and acquiring basic need items.

• Assist clients with issues related to benefits, housing, childcare, healthcare, and other recovery supports that might serve to support treatment success.
• Coordinate client care with other health care providers (primary care doctors, treatment counselors, etc.).
• Keep up-to-date accurate electronic case notes in accordance with rigid DOH standards.
• Make appropriate referrals and provide follow-up.
• Complete assessments and supporting documentation in a timely fashion.
• Able to effectively utilize on-line Health Homes care management system.
• Assist with Open Access care management when emergent issues arise, coordinating with Open Access staff on accessible resources prior and post treatment placement.
• Perform other tasks as assigned.

DESIRED SKILLS/ABILITIES:

• Experience as a Health Home Care manager is strongly desired.
• Excellent computer skills and ability to utilize a complex, web-based care management system.
• Familiar with resources in the community that meet a wide variety of client needs.
• Ability to successfully engage clients.
• Comfortable working independently.
