Fatherhood Community Health Worker (CHW/FT)

Position Summary:

The Fatherhood Community Health Worker (CHW) provides care coordination, referrals and follow-up, and birth and parenting education for Fathers to achieve appropriate child health, social, and emotional development, and milestone achievement. Reports to the Fatherhood CHW Lead.

Minimum Qualifications:

  • Associate degree preferred in human services field or 2-year equivalent professional experience
  • Demonstrates commitment to developing and supporting Fatherhood at individual and community levels
  • Experience coaching individuals and/or groups
  • Knowledgeable in perinatal health and human service agencies within the community, as well as Fatherhood specific resources
  • Commitment to culturally responsive care, and understanding Fatherhood as a social determinant of health
  • Experience working with racially, ethnically, and culturally diverse populations in the inner city of Rochester
  • Ability to acknowledge, discuss, and address racial equity issues impacting health
  • Community affiliations with existing efforts to address Fatherhood and/or supporting Fathers
  • Strong written, verbal, interpersonal, organizational, and problem-solving skills
  • Fluent in use of the internet and Windows-based computer systems
  • Ability to work flexible hours, including occasional evening and weekend hours
  • Valid NYS Driver's License and reliable transportation

Duties and Responsibilities:

  • Communicate effectively with Fathers, their Partners, Families, Support System, and agency staff
  • Identify, recruit, and engage Fathers to assess client requests for support and refer appropriately
  • Strengthen Fatherhood engagement and retention through outreach, recruitment, and relationship building
  • Document in electronic system outreach attempts, referrals, and follow-up
  • Travel to client homes and various community and outreach locations
  • Maintain knowledge of local human service agencies and community resources
  • Maintain a robust understanding of Rochester neighborhoods, assets, and challenges
  • Always demonstrate a high level of professionalism
  • Represent agency and program at relevant community collaborations and events
  • Additional duties or projects as assigned

Salary Range: $39,000
